Functional Testing for Perimenopause and Menopause: A Root-Cause Approach to Hormonal Health
By Remi Odisho
Perimenopause and menopause are often framed as inevitable hormonal milestones that women simply have to “push through.” Many women are told their symptoms are normal, their blood tests are fine, or that hormone replacement is the only option available.
Functional medicine takes a different view.
From an integrative and functional medicine perspective, perimenopause and menopause are complex whole-body transitions influenced by stress, gut health, adrenal function, inflammation, and detoxification pathways — not just declining oestrogen levels.
Functional testing allows us to identify the drivers of symptoms and create targeted, personalised treatment plans that support women through this stage of life.
Common Perimenopause and Menopause Symptoms
Women often seek support for a wide range of symptoms, including:
- Hot flushes and night sweats
- Anxiety, low mood, or irritability
- Poor sleep or early morning waking
- Fatigue and burnout
- Brain fog and poor concentration
- Weight gain or changes in body composition
- Heavy, irregular, or painful periods (perimenopause)
- Low libido and vaginal dryness
While these symptoms are common, they are not something you have to tolerate.
Why Standard Hormone Tests Often Miss the Full Picture
Conventional blood tests typically measure hormone levels at a single point in time. During perimenopause — when hormones fluctuate dramatically — this snapshot approach may not reflect what is happening day to day.
Standard testing often does not assess:
- Daily cortisol patterns and stress response
- Hormone metabolism and clearance
- The interaction between gut health and oestrogen recycling
- Tissue-level hormone activity
Functional testing provides deeper insight into how hormones are being produced, used, and cleared in the body.
Stress, Cortisol, and Hormonal Imbalance
Chronic stress is one of the most significant drivers of perimenopausal and menopausal symptoms.
When the body is under ongoing stress, it prioritises cortisol production to support survival functions such as blood sugar regulation and inflammation. The building blocks used to make cortisol are also required to produce sex hormones — particularly progesterone.
This phenomenon is often referred to as “cortisol steal.”
When cortisol demand is high:
- Progesterone levels commonly decline first
- Oestrogen may become dominant relative to progesterone
- Symptoms such as anxiety, sleep disruption, breast tenderness, heavy periods, and mood changes may worsen
How Functional Testing Helps
Functional hormone testing can assess:
- Cortisol output across the day
- Stress resilience and adrenal patterns
- The balance between cortisol, progesterone, and oestrogen
This allows treatment to focus not just on hormones, but on stress regulation, nervous system support, sleep quality, and lifestyle interventions that restore hormonal balance.
Gut Health and the Menopause Transition
The gut plays a critical role in hormone balance, particularly during midlife.
Certain gut bacteria are responsible for metabolising and recycling oestrogen — a system known as the estrobolome. When gut health is compromised, oestrogen may be poorly cleared or excessively reabsorbed, contributing to hormonal symptoms.
Poor gut function may be associated with:
- Oestrogen dominance symptoms (painful/heavy periods, breast tenderness, brain fog)
- Increased inflammation and joint pain
- Worsening hot flushes and mood changes
- Bloating, reflux, or altered bowel habits
Functional Gut Testing Can Assess:
- Gut microbiome balance and diversity
- Inflammation and gut permeability
- Infections or dysbiosis
- Markers related to oestrogen recycling
Supporting gut health often leads to significant improvements in hormonal symptoms, even without direct hormone therapy.
Detoxification and Hormone Clearance
As ovarian hormone production declines, the body becomes more reliant on liver and gut detoxification pathways to metabolise hormones efficiently.
If these pathways are overwhelmed or under-functioning, hormone metabolites can contribute to inflammation and symptom severity.
Functional testing can evaluate:
- Oestrogen and progesterone metabolites
- Detoxification pathways
- Nutrient cofactors required for optimal hormone clearance
This allows for targeted nutritional and lifestyle strategies that support the body’s natural hormone processing systems.
A Personalised, Integrative Approach to Menopause Care
Functional testing enables us to move beyond symptom suppression and instead address the root causes of hormonal imbalance.
By understanding how stress, gut health, detoxification, and hormones interact, we can create individualised treatment plans that may include:
- Targeted nutrition and supplementation
- Stress and nervous system regulation
- Gut healing protocols
- Lifestyle and sleep optimisation
- Hormonal support when clinically appropriate
Supporting You Through Perimenopause and Menopause
Perimenopause and menopause are not conditions to be endured — they are transitions that deserve informed, compassionate, and personalised care.
Functional testing provides clarity, validation, and direction for women who want to feel well, energised, and supported through midlife and beyond.
Book a Functional Hormone Consultation
If you are experiencing perimenopause or menopause symptoms and want a deeper understanding of what is happening in your body, I would love to support you.
As an integrative medicine practitioner with a special interest in female hormones and reproductive health, I work closely with women to investigate the root causes of hormonal symptoms using functional testing and personalised care.
You can book a functional hormone consultation with me to explore whether functional hormone, stress, or gut testing is appropriate for you, and to develop an individualised plan that supports you through perimenopause and menopause.
Appointments can be booked here or you are welcome to contact the clinic if you have any questions about working with me or functional testing.
